It is hard to believe that another school year will be ending in a couple of months. I wanted to let all the 6th grade parents know of the immunization requirements for all 7th graders. The school system is required to have an updated Immunization Form #3231 on file for all 7th graders because they are required to have vaccinations for adolescent pertussis (whooping cough) (Tdap) and Meningococcal (MCV4). This policy is set forth in policy guide 3231 REQ, Vaccine Requirements for Attending Facilities and Schools in Georgia. It will be necessary for parents to take their upcoming 7th graders to either their primary care provider or local health department for the vaccinations. 

If parents do not want to get their child vaccinated, which I do not suggest, then a (DPH form 2208) Affidavit of Religious Objections to Immunizations needs to be filled out. This form is provided by the Georgia Department of Public Health and will need to be signed by parents and notarized. 

The Schley County School System is audited by the local health department each fall to make sure we are in compliance with this law. If your child already has these vaccinations and you have an Immunization Form #323l that has the 7th grade or higher block checked, which is located in the upper right corner of the form, then these can be brought in and given to the nurse.
